Probation Reporting Form

The Westminster Probation Office supervises Westminster Municipal Court cases only.

The probation officers supervise those assigned to probation by the Court throughout the probation period to assure compliance with the terms and conditions imposed as part of the sentence.  These terms and conditions may include attendance at classes, counseling, drug testing, evaluations, and any other terms or conditions deemed appropriate by the Court. Many probationers are required to submit random UA tests (urine analysis).

If you have been ordered to submit random UA's, please contact Sentry UA System at 303-515-7619 daily.  You will need your probation identification (I.D.) number.  Please contact the Probation Office or your Probation Officer directly if you do not know your I.D. number. 

Failure to comply with the probation terms and conditions may result in the revocation of your probation and the imposition of a sentence, which could include jail time. Please contact your Probation Officer or report to the Probation Office immediately if any circumstances arise that may impair your ability to comply with the terms and conditions of your probation . Please also contact the Probation Office if you have any questions or need any further clarifications about the orders imposed by the Court.  

The Court could also order the Probation Office to prepare a pre-sentence investigation report about a defendant prior to sentencing. In this case, a probation officer interviews the defendant, checks their prior criminal record and personal background, and provides a sentencing recommendation to the Court.
