Please Note: Due to the COVID-19 pandemic, the City has suspended Passport Services until 2022. For updates from the US Department of State regarding the effect of COVID-19 closure on their processing of passports visit their website.

Westminster City Hall, located at 4800 W. 92nd Avenue, provides passport services for:

  • NEW passport applications for adults and children
  • RENEWALS for children
  • Appointments are required and occur Tuesday through Thursday.
  • Please call 303-658-2337 to make an appointment.
  • Adult Renewals are NOT handled at Westminster City Hall. Please visit the U.S. Department of State's website.

Passport applications must be completed prior to your appointment. Passport forms are available at the City of Westminster City Hall or online at the U.S. Department of State's website.

All necessary documents and the correct form of payment must be provided at the time of the appointment or you will be asked to make a new appointment. You will need to bring:

  • A "completed" DS11 Form (DO NOT sign the form - You will need to sign in front of the Passport Agent)
  • Passport Applicants 16 and older must have a valid, state-issued photo ID; preferably a driver's license
  • Proof of U.S. Citizenship (only originals will be accepted - no photocopies); this will include one of the following:
    • State issued birth certificate filed within one year of birth with both parents names listed
    • Naturalization Certificate
    • Expired adult U.S. passport
  • One passport photo (photos can be taken at Costco, Walgreens, Walmart or FedEx - please contact these companies directly for costs)
  • All passport applicants must be present at the time of the appointment
  • Children 15 and under must have BOTH parents present with an ID. If one parent cannot be present, Form DS-3053 needs to be filled out by the non-appearing parent, notarized and accompanied by a photocopy of the front and back of that parent's ID.   • Children 16-17 years of age must have one parent present with an ID.
  • Payment of Fees - Checks, Money Orders or Cashier's Checks ONLY.  (Please call for the correct amounts.  Fees are split into two payments:  one payment to the U.S. Department of State and one payment to the City of Westminster.)

Current processing of a new passport is six (6) to eight (8) weeks. Passport applications may be expedited for an additional fee. If you require your passport within two (2) weeks, visit the U.S. Department of State's website.

Important notes about passport expiration dates:

  • The U.S. Department of State recommends your passport have at least six (6) months of validity beyond your actual travel date to avoid expensive and lengthy travel disruptions. The normal rule for most countries is that they require you to have three (3) months from your actual travel date before your passport expires.
  • U.S. citizens traveling on passports that expire in fewer than six (6) months have increasingly been denied airline boarding or have been detained upon arrival in certain foreign destinations, including popular European travel destinations in the Schengen area.  
  • This is not a a new requirement, but it is only until recently that the requirement has been more strictly enforced.
